I have an 04 LX470 and did the fix found here and works great. From memory, I recall the menu button being an actual button (drivers side) but the volume is just a knob and what exactly do you mean by "top left of the screen, bottom left, top left etc"... are these buttons on the Nav screen? It's great that you can enable this while in motion and it's set (until you cycle the ignition). I also saw that there's other functions, but didn't mess with them.
