It's happened 4 times on my 2007 IS250. Two times it was shop induced after maintenance...Mass Airflow Sensor left disconnected and rear axle speed sensor left disconnected. Once it latches, the code can only be reset after the connectors were reinstalled. It does not reset itself. It can be with a OBD code reader or by disconnecting and reconnecting the battery (Basically rebooting the computer). The other two are unknown but did reset.
