So a friend figured out the problem and fixed it. Hopefully this will help someone somewhere.
After replacing bulb, ballast and fuses there were little options left. A friend offered to take a look and carefully observed all connections. Upon inspection, he found that right where the bulb meets the plug there was a little black mark and tiny piece of melted plastic. This led him to believe that there was a bad connection causing heat. It turns out the bulb had a dent where the metal connector is supposed to go and was causing a bad connection. This dent is a design flaw from aftermarket bulbs. Replacing with Sylvania high quality bulbs resolved the issue and both headlights are working fine for a few days now.
