2001 Es300 Blower Motor Noisy/quit/came Back On

The blower motor on 2001 ES300 has been a bit loud and yesterday quit and then came on after I shut off and turned back on a few times while the car was moving about 60 mph The air conditioning works as I can hear the compressor kick on. Possible causes and or/fixes?

If the blower is noisey, chances are that the motor is worn out. How many miles is on the car?
