High Rpm When Shifting

I have a '99 RX 300.

When i accelerate the RPM increases to 4-5k and gets stuck there.

The RPM won't reduce even when i take my foot off gas but after a while (5 - 10 secs) it comes back down to 2.5 - 3k. It seems like this happens when it

is shifting to a higher gear.

What i have also noticed is sometimes the RPM is very low, around the 1k mark and i have to hit the gas for the car to move. This happens when i move from

a stand still and has occurred 2-3 times in the last 2 weeks.

I started noticing this issue from the past 3-4 weeks. Has anyone had a similar experience. Appreciate any input on this.
