We had this problem in our 2006 400h and found a faulty amplifier (under & behind the rear seat). We had this diagnosed & replaced under the Certified Pre-owned Warranty.

Steve, The fluid should be changed every so often. I don't have the information available right now, but I believe Lexus recommends every 60,000 miles. I would recommend every 30,000 miles. The fluid should come from Lexus or Toyota. I think the grade is ATF-WS or something like that, the WS standing for "World Standard" Mike
