My 2007 ES350, almost 30k miles, has the same squeak issue when it's back up, and it usually happens in the rainy days. I had it fixed by the Lexus dealer in Fremont CA last Friday. They adjusted the brake position, smoothed the rotors, and also installed 4 new pads. Now it works great. It's covered by warranty, no charge at all.

My 2007 ES 350 has the similar issue. RPM may surged if I decelerated to 20~30 and then accelerated. It happened a couple of times a week. Two weeks ago I had 10K service, and fortunately I showed them this transmission issue. Then they fixed. When I picked up my car, I found it's actually a recall for shift flare concern. After that, It never happened again, even if I tried to replicate it. So I think this issue is resolved. Hope this is helpful.

My car bought at March 2007 has the same issues as you described, except that I didn't experience any 3rd to 4th gear flare. I also have the issue of braking noise. As I slow down to a stop, there is a kind of rumble just before stop. This only happens at low speed but not at high speed. I mentioned it to the Lexus dealer in Fremont. They checked and said it's very normal. But I don't have this issue for my BMW325i and Honda Accord. Is this really normal for braking noise? Please comments!
