On my Rx400h it also usually stays at least 2 bars below max. For example, I experimented to drive in a mountain, descending during 50 miles without accelerating at all, and I finaly saw one green bar! The Lexus specialist tested everything, but all is normal he said.

RX400h has only 3000 miles, and today, after a 2h shopping, my wife found her car with a dead battery. I don't understanf how Lexus can stay unactive with such frequent issues, even if all RX400h are not affected. Towing the car from Ajaccio to Bastia (Corsica island, in France), car rental for my wife, repair and towing back to Ajaccio will cost them more than 1000 USD! How can this pb being not solved ?
