Damage After Removing Key In Accessory Position

Has anyone had any experience with this problem. I loaned my car to someone and they somehow removed the key while in the accessory position. I came behind them and was unable to start the car. The door chime rang as if the key were still in the engine. The car ended up being towed, and a new ignition cylinder and ignition housing had to be replaced. The key was fine, and was not replaced. I don't understand. Was this a wear and tear type problem? I've never heard of it before, and would be curious if others have had a similar experience. Obviously, the car was not under warranty. I was just curious what other reasons could cause this to happen.
