Recently on a long drive with my family, I had to stop at a friends place, and on departure, the smart key just did not work, no crank or neither start, just dead, Had to call lexus and just collected my wife's car 3 days back, Guess what they replaced the unit for free, I was surprised, so I had asked the manager what was the issue,and he mentioned that they had to replace certain models, as a TSB has been issued in Nov 08 for such issues. I managed to get one copy, Hope it is useful to others who could face such problems.

Yep, modules go out from time to time. I hate the inconveniance it causes. Haven't seen one go out in a long time tho
